RAI Manual v1.20.1Steps for Assessment
1. Vaccination status may be determined based on information from any available source.
•
Review the resident’s medical record or documentation of COVID-19 vaccination and/or
interview the resident, family or other caregivers or healthcare providers to determine
whether the resident is up to date with their COVID-19 vaccine.
2. If the resident is not up to date, and the facility has the vaccine available, ask the resident if
they would like to receive the COVID-19 vaccine.
