3. Plan-of-care goals

The overlay goals, tied to the ordered parameters above, are:

  • The patient and caregiver recognize an exacerbation early — by the three cardinal symptoms — and act on the written action plan before hospitalization becomes necessary.
  • Oxygenation is maintained within the ordered range on the ordered flow, without over-oxygenation.
  • Inhaler and oxygen technique are correct on observation, so the prescribed therapy is actually delivered.
  • Rescue-medication use stays within the ordered frequency, and escalation is recognized and reported.
  • Exacerbation frequency and hospitalizations decrease over the episode of care, with the GOLD group and exacerbation history documented and reassessed.
