Behavioral health is a major nursing-home survey focus, and the misuse of psychotropic medications — especially antipsychotics in residents with dementia — is among the most cited and publicly reported concerns. The facility must provide the behavioral-health services each resident needs (F740–F745) and ensure each resident’s drug regimen is free of unnecessary drugs, with psychotropics used only for a documented indication, with monitoring, within PRN time limits, and with gradual-dose-reduction (GDR) considered (F758).
The documentation must show non-pharmacologic interventions first, a valid indication and target symptoms when a psychotropic is used, ongoing behavior monitoring, and the GDR schedule or the clinical rationale for not reducing.
