Skilled interventions
- Implement and verify the individualized fall-prevention interventions (alarms, rounding, toileting schedule, footwear, environment, devices) each shift.
- Perform the post-fall assessment: injury survey, neuro checks per protocol, orthostatic vitals, pain, and functional screen.
- Add a new, specific intervention after every fall and document the root cause — a fall with no plan change is a deficiency.
Teaching / training (to mastery)
- Teach the resident/representative the fall-prevention plan and the call-don’t-fall message, with teach-back.
- Train CNAs on the specific interventions (transfer technique, toileting schedule, alarm use) with return-demonstration.
