5. Skilled interventions and teaching

Skilled interventions

  • Assess pain with the appropriate scale each shift and before/after interventions; reassess and document the effect with timing.
  • Deliver pharmacologic and non-pharmacologic interventions per the individualized plan; monitor opioids (sedation, respirations) and maintain the bowel regimen.
  • Escalate uncontrolled or new pain to the provider and update the plan.

Teaching / training (to mastery)

  • Teach the resident/representative the pain plan, realistic goals, and how to report pain, with teach-back.
  • Train CNAs to recognize non-verbal pain cues and report them, with documented understanding.
