Post-craniotomy complication

New worsening headache with nausea and increasing drowsiness (or incision drainage/erythema) — neuro/incision assessed, neurosurgery/provider notified urgently, imaging/evaluation ordered, representative notified, close neuro monitoring; emergency; supports Section I.

Delayed bleed on anticoagulant

Post-head-strike on apixaban with new headache, one vomit, increasing drowsiness — EMS/transfer activated, anticoagulant status communicated, provider/representative notified, neuro status/time documented; delayed intracranial bleed. (Falls/Anticoag.)

Scalp-wound infection

Scalp laceration repair with spreading erythema, purulence, low-grade fever — provider notified, culture/antibiotics and wound care ordered, systemic monitoring; supports Section M/I; skilled recognition.
