Acute psychiatric crisis — severe agitation, psychosis, or danger to self/others — ensure safety and escalate per the crisis pathway (route to the Behavioral reference).

Signs of abuse, neglect, or exploitation — follow the facility reporting policy and required timelines (a facility-policy obligation outside this documentation module).

Caregiver breakdown threatening a safe discharge — escalate to social services and the team; do not proceed with an unsafe discharge.
