Pain that is under-treated robs patients of function and dignity; pain therapy that is unsafe — particularly opioids — causes sedation, respiratory depression, constipation, and overdose. The skilled value of this overlay is holding both truths at once: controlling pain to a functional goal while monitoring the safety parameters that predict opioid harm — sedation and respiratory rate — keeping naloxone available, and preventing opioid-induced constipation, all documented so any nurse can act on it.
This overlay supplements the Pain assessment. The full pain evaluation — location, quality, intensity, and pattern — lives in that module; here the clinician adds the functional pain goal, the sedation and respiratory-rate safety monitoring, naloxone readiness, bowel management, and safe-use teaching, pinned to the current CDC opioid-prescribing guidance. Sedation is monitored deliberately because increasing sedation precedes opioid-induced respiratory depression.
