3. Plan-of-care goals

The overlay goals, tied to the ordered parameters above, are:

  • Pain is controlled to the individualized functional goal, so the patient can perform the ordered target activities.
  • The opioid-safety parameters stay within limits — no excessive sedation and a respiratory rate above the ordered threshold — and any deviation is recognized and acted on.
  • Naloxone is present, in date, and the caregiver is trained in its use.
  • Opioid-induced constipation is prevented, with a bowel movement within the ordered interval.
  • Medications are used, stored, and disposed of safely, and function and comfort improve over the episode of care.
