Skilled need not articulated for a stable patient

  • Fails: A stable, self-managing patient charted with no stated ongoing skilled need — no instability, teaching gap, or management rationale.
  • Survives: The specific reason skilled care remains required — surveillance for a named risk, an unmet teaching goal, or a management/evaluation need tied to the diagnosis and orders.

The common fix

Every pattern above resolves the same way: state the actual finding and its objective reference (never “at baseline”), name the RN-level reason a nurse was required, quantify the effect, and close the loop. Applied consistently, these are the disciplines the Foundations module teaches and the Compliance checklist enforces.
