The two-condition rule Medicare requires both: (1) the patient needs assistance or has a condition contraindicating leaving home, and (2) leaving requires a considerable and taxing effort. Each must be documented with patient-specific evidence at every visit. |

- The specific condition or assistance need — a named diagnosis or assistive device.
- Concrete evidence of the taxing effort — distance, time, recovery, helper.
- Frequency or recency of leaving home — not “never leaves,” which overclaims.
