Track intake/output and daily weight and assess volume/electrolyte status with parameter-based escalation.
Assess and protect the dialysis access (thrill/bruit; no BP/venipuncture in the access limb) and coordinate the dialysis schedule and post-dialysis assessment.
Administer and monitor renal medications and the renal diet and document the response (route dialysis specifics to the Dialysis overlay).
Teaching / training (to mastery)
Teach the resident/representative fluid/sodium/potassium and phosphate management, access protection, the dialysis schedule, and warning signs, with teach-back.
Train CNAs on accurate intake/output and weights, access-limb protection, and prompt reporting, with documented understanding.
