6. Red flags — escalate / transfer

🚩 Red flags

Pulmonary edema / severe fluid overload — severe dyspnea, hypoxia, distress in renal impairment — emergency evaluation/transfer.

Life-threatening hyperkalemia — weakness, arrhythmia, or a critical potassium — escalate/transfer per protocol.

Dialysis-access loss or hemorrhage — a lost thrill/bruit, or bleeding from the access that does not stop with pressure — urgent notification/transfer.

Uremic emergency — marked confusion, pericardial signs, or severe symptoms — escalate/transfer.
