6. Red flags — escalate / transfer

*🚩 Red flags*
*Pulmonary edema / severe fluid overload* *— severe dyspnea, hypoxia, distress in renal impairment — emergency evaluation/transfer.* *Life-threatening hyperkalemia* *— weakness, arrhythmia, or a critical potassium — escalate/transfer per protocol.* *Dialysis-access loss or hemorrhage* *— a lost thrill/bruit, or bleeding from the access that does not stop with pressure — urgent notification/transfer.* *Uremic emergency* *— marked confusion, pericardial signs, or severe symptoms — escalate/transfer.*
