1. Purpose and clinical relevance

The constitutional view — fever, energy, appetite, weight, and overall function — is where a change in an older adult often shows first, before any single system localizes it. The skilled work is systemic surveillance (especially fever and early sepsis), the recognition of a generalized decline or failure to thrive, and the management and evaluation of a complex, multi-problem plan where the integrating judgment is itself the skilled service.

The purpose is to prove, for each billed day it applies, that skilled systemic observation or management and evaluation of the plan was reasonable, necessary, needed daily, and available only in a SNF under Pub. 100-02 Chapter 8. The constitutional assessment is the same as in home care; the coverage spine is re-pointed, and the management-and-evaluation basis is made explicit.
