Skilled interventions

  • Perform systemic/constitutional surveillance and early sepsis screening with parameter-based escalation.
  • Recognize generalized decline/failure to thrive and re-evaluate the multi-problem plan and its reversible causes.
  • Provide management and evaluation of the interacting problems and verify the unskilled care is achieving its purpose.

Teaching / training (to mastery)

  • Teach the resident/representative the warning signs of a systemic change (fever, decline) and the plan, and revisit goals of care as appropriate, with teach-back.
  • Train CNAs to report early constitutional changes (fever, reduced intake/energy, functional decline) promptly, with documented understanding.
