New delirium

Acute new disorientation and lethargy with fluctuating attention and disorganized thinking (CAM positive), different from the dementia baseline — delirium workup per order, provider/representative notified, safety measures; reassess; evaluate SCSA; supports Section C.

Hyperactive delirium, trigger screen

New agitation with a low-grade fever and reduced intake — screened for a medical trigger, provider notified, workup ordered, non-pharm measures, unnecessary psychotropic avoided; reassess; supports Section C. (Behavioral reference.)

Delirium from a dangerous cause

New confusion with a positive CAM and hypoglycemia on the ordered check — treated per protocol, provider notified, recheck and evaluation ordered, monitoring; cognitive change tied to the corrected cause; supports Section C.
