The skilled work is daily and specific: obtaining and interpreting a daily weight, correlating weight and edema and lung findings with the diuretic order, titrating within the ordered parameters, monitoring guideline-directed medical therapy (an ACE/ARB/ARNI, a beta-blocker, an MRA, and an SGLT2 inhibitor) for tolerance, and catching decompensation before it becomes a readmission.
This overlay proves, for each billed day, that skilled HF observation, management and evaluation of the plan, teaching, or a covered procedure was reasonable, necessary, needed daily, and available only in a SNF under Pub. 100-02 Chapter 8. Use it with the Cardiovascular reference, which supplies the base cardiac exam and the general arrhythmia/ACS content.
