5. Skilled interventions and teaching

Skilled interventions

  • Obtain and trend the daily weight; apply the parameter-based diuretic order and document the weight/edema/symptom response.
  • Monitor GDMT tolerance (BP, HR, renal function, potassium) and administer/hold per parameters; correlate congestion findings with the plan.
  • Provide decompensation surveillance with parameter-based escalation and coordinate labs (BMP/BNP) as ordered.

Teaching / training (to mastery)

  • Teach the resident/representative daily-weight monitoring and the ‘call parameters’ (e.g., a defined weight gain or new dyspnea), the sodium/fluid plan, GDMT purpose and adherence, and the symptom-action plan, with teach-back.
  • Train CNAs on accurate daily weights, edema/intake reporting, and prompt reporting of dyspnea or a weight change, with documented understanding.
