Assess sensation and protective sensation and inspect the at-risk skin/feet for unnoticed injury; provide active protective management (footwear, offloading, positioning, temperature safety).
Recognize an acute or ascending sensory change and escalate; account for the deficit in fall-safety and mobility.
Revise the protective plan after any injury from lost sensation and coordinate wound care (route to Skin).
Teaching / training (to mastery)
Teach the resident/representative daily skin/foot inspection, temperature safety (checking water/heat), protective footwear, and the warning signs, with return-demonstration/teach-back.
Train CNAs on protecting insensate areas, inspecting the feet/skin, and reporting any new injury or a sensory change, with documented understanding.
