Scenario

Model daily note (edit to the shift)

Surgical-site infection

Incision with spreading erythema, warmth, purulent drainage, low-grade fever — provider notified, culture and possible antibiotics ordered, dressing changed, limb/systemic signs reassessed, care plan revised; supports Section I/N; skilled recognition daily.

Suspected DVT

New unilateral calf swelling, warmth, tenderness, calf pain on movement — provider notified, limb assessed/measured, prophylaxis and activity reviewed, Doppler ordered, embolic monitoring; reassess; skilled surveillance (time-critical). (Anticoag overlay.)

Prosthetic dislocation after a fall

After a fall, sudden severe hip pain with the leg shortened/internally rotated and unable to bear weight — immobilized, limb not moved, vitals obtained, SBAR, transfer order, representative notified; fall coded Section J; care plan revised on return.
