Skilled interventions

  • Perform ordered incision/wound care and assess the surgical site and operative-limb neurovascular status each shift.
  • Enforce and document weight-bearing status and surgical precautions; administer and monitor VTE prophylaxis.
  • Coordinate pain management to enable therapy and deliver/evaluate rehabilitation-nursing carryover (route program detail to Restorative).

Teaching / training (to mastery)

  • Teach the resident/representative the surgical precautions, weight-bearing status, incision care, VTE warning signs, and safe transfer/mobility technique, with return-demonstration/teach-back.
  • Train CNAs on the precautions, safe transfer/assist levels, and early reporting of wound or limb change, with return-demonstration.
