Skilled interventions

  • Monitor glucose per order, administer insulin with sliding-scale/titration judgment, and document the response.
  • Recognize and treat hypo/hyperglycemia per protocol with a documented recheck; obtain ketones/labs and coordinate care as ordered.
  • Keep diabetic complications on the skilled radar (foot, renal) and coordinate the diet and activity with the insulin plan.

Teaching / training (to mastery)

  • Teach the resident/representative glucometer use, insulin technique, the 15-15 hypoglycemia rule, sick-day guidance, foot care, and warning signs, with return-demonstration/teach-back.
  • Train CNAs on hypoglycemia recognition, timely meal delivery relative to insulin, foot inspection, and prompt reporting, with documented understanding.
