1. Purpose and clinical relevance

Pressure injuries are a sentinel indicator of nursing-home quality and a frequent source of citations and litigation. The facility must ensure a resident who enters without a pressure injury does not develop one unless unavoidable, and that a resident with one receives care to promote healing and prevent new injuries (F686). The documentation must show risk-based prevention, accurate staging and measurement, the present-on-admission determination, and the healing trajectory.

Skilled wound care — a complex or sterile dressing change, and the assessment of a healing trajectory — is a covered daily skilled service under Chapter 8.
