Skilled interventions

  • Perform the ordered wound care with correct (sterile/clean) technique; document the wound assessment and measurements each dressing change.
  • Implement and verify the prevention bundle: repositioning schedule, support surface, heel offloading, moisture management, and nutrition/hydration support.
  • Determine and document present-on-admission status on admission; determine avoidable vs unavoidable for facility-acquired injuries.

Teaching / training (to mastery)

  • Teach the resident/representative repositioning, offloading, and nutrition’s role in healing, with teach-back.
  • Train CNAs on repositioning technique, heel floating, and early skin-change reporting with return-demonstration.
