Skilled interventions

  • Assess the perineal/genital skin and provide specialized moisture-barrier, containment, and pressure management; distinguish moisture damage from a pressure injury.
  • Provide complex perineal wound care per order with periwound protection and containment; trend the wound (route staging to Skin).
  • Recognize local infection and, critically, necrotizing infection (Fournier gangrene) and escalate emergently.

Teaching / training (to mastery)

  • Teach the resident/representative perineal skin care, moisture management, and the warning signs of breakdown or infection, with return-demonstration/teach-back.
  • Train CNAs on prompt incontinence cleansing, barrier application, pressure offloading, and reporting new erythema, breakdown, drainage, or a dusky/spreading area, with return-demonstration.
