Daily-note bank — change-of-condition / at-risk days

Scenario

Model daily note (edit to the shift)

Anticoagulated epistaxis

Active brisk nosebleed uncontrolled with initial pressure on warfarin — continuous pressure and airway positioning, vitals monitored, anticoagulation reviewed/next dose per parameters, SBAR, evaluation/packing/labs ordered, representative notified; reassess; supports Section I. (Anticoag overlay.)

CSF rhinorrhea

New clear watery drainage from one nostril post-transsphenoidal — neurosurgery/provider notified urgently, HOB per order, no nose-blowing, drainage characterized, neuro monitored; meningitis risk; supports Section I. (Head/Neuro.)

Post-op sinus infection

Post-sinus-surgery with fever, purulent drainage, and facial pain/swelling — provider notified, culture/antibiotics ordered, systemic monitoring; supports Section I; skilled recognition.

Linked disease-specific scenarios

Oxygen delivery and respiratory device detail route to the Respiratory reference; NG feeding routes to the Nutrition reference; anticoagulation-related bleeding routes to the Anticoagulation overlay; a CSF leak and the neurosurgical course route to the Head/Scalp and Neurological references.
