Document each teaching point with topic, method, audience, and measured mastery (teach-back percentage or return-demonstration result), per Foundations — Teach-back and return demonstration.

  • Teaching delivered in the patient's preferred language and adapted to the communication need.
  • Caregiver communication strategies for a patient with a speech, hearing, or cognitive-communication impairment.
  • Correct use of hearing aids, corrective lenses, and augmentative devices.
  • Using interpreters and available language resources.
  • Confirming understanding through teach-back at each teaching interaction.
