Abnormal findings and what they may signal — each warrants Mode B focused documentation and, where parameters are met, intervention and notification:
- Speech impairment — dysarthria or expressive or receptive aphasia.
- A language barrier with limited English proficiency and an interpreter need.
- Functional hearing impairment that affects comprehension of instructions.
- Functional vision impairment that affects reading of materials.
- Low health literacy with limited comprehension of health information.
- Cognitive-communication impairment affecting understanding.
- An unmet augmentative-communication need, or any barrier that compromises understanding, safety, or consent.
