5. Abnormal findings

Abnormal findings and what they may signal — each warrants Mode B focused documentation and, where parameters are met, intervention and notification:

  • Speech impairment — dysarthria or expressive or receptive aphasia.
  • A language barrier with limited English proficiency and an interpreter need.
  • Functional hearing impairment that affects comprehension of instructions.
  • Functional vision impairment that affects reading of materials.
  • Low health literacy with limited comprehension of health information.
  • Cognitive-communication impairment affecting understanding.
  • An unmet augmentative-communication need, or any barrier that compromises understanding, safety, or consent.
