10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

• Demonstrated comprehension after an accommodation — a successful teach-back through an interpreter or with adapted materials.
• The effectiveness of an arranged accommodation or device.
• Teaching effectiveness: the teach-back or return-demonstration result with the accommodation in place.
• Whether the barrier is resolved or what remains to be arranged.
• The plan for reassessment timing and what will be re-evaluated next visit.
