Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.
- Demonstrated comprehension after an accommodation — a successful teach-back through an interpreter or with adapted materials.
- The effectiveness of an arranged accommodation or device.
- Teaching effectiveness: the teach-back or return-demonstration result with the accommodation in place.
- Whether the barrier is resolved or what remains to be arranged.
- The plan for reassessment timing and what will be re-evaluated next visit.
