Measure

  • A validated cognitive screen result (Mini-Cog or MoCA).
  • The Confusion Assessment Method (CAM) when an acute change suggests delirium.
  • The comparison of the screen and the clinical picture to the patient's baseline.
  • Vital signs and a review for medical precipitants when an acute change is present (interpreted with Vital Signs and General / Constitutional).
