4. Normal findings

The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.

  • Alert and oriented to person, place, time, and situation at the patient's baseline.
  • Recent and remote memory at baseline.
  • Attention and concentration intact.
  • Executive function and judgment at baseline.
  • A stable level of consciousness with no acute change or fluctuation — no delirium features.
  • Cognition supports safe function and medication management.
  • A validated screen at or near the patient's baseline.
