Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.
- The effect of an intervention — for example improvement after a precipitant is treated, or a reduction in agitation with supportive strategies.
- Caregiver competence with cognitive-supportive and medication-supervision strategies.
- Teaching effectiveness: teach-back of delirium recognition and the supportive strategies.
- Whether cognition has changed and what it indicates.
- The plan for reassessment timing and what will be re-evaluated next visit.
