Symptom screen: orthopnea (number of pillows), paroxysmal nocturnal dyspnea, exertional tolerance, and nocturnal cough.

Guideline-directed therapy adherence and tolerance (diuretic, ACE-I/ARB/ARNI, beta-blocker, mineralocorticoid-receptor antagonist, SGLT2 inhibitor): effect, adverse effects, and labs due.
Fluid and sodium intake review; an orthostatic check if the patient is on diuretics or is symptomatic.
