3. Key visit elements

Add these heart-failure-specific elements at each visit alongside the cardiovascular and respiratory examinations — this is not a repeat of the full exam:

  • Daily-weight trend reviewed against the ordered dry weight and the report threshold.
  • Volume signs (jugular venous distension, edema versus baseline, lung crackles, S3) are documented in the parent modules; here, record the trend and the zone determination.
  • Symptom screen: orthopnea (number of pillows), paroxysmal nocturnal dyspnea, exertional tolerance, and nocturnal cough.
  • Zone determination (green / yellow / red) and the action taken.
  • Guideline-directed therapy adherence and tolerance (diuretic, ACE-I/ARB/ARNI, beta-blocker, mineralocorticoid-receptor antagonist, SGLT2 inhibitor): effect, adverse effects, and labs due.
  • Fluid and sodium intake review; an orthostatic check if the patient is on diuretics or is symptomatic.
