4. Medication and treatment monitoring

Monitor the disease-specific therapy for effect and adverse effects; report values outside the ordered parameters. Exact monitoring intervals follow the provider's orders.

  • Diuretics (loop ± thiazide): weight and volume response, electrolytes (potassium, sodium), renal function, and signs of volume depletion or orthostasis; use of any PRN diuretic parameter.
  • ACE inhibitors / ARBs / ARNI: blood pressure, renal function, and potassium; an ARNI requires a 36-hour washout from an ACE inhibitor because of angioedema risk.
  • Beta-blockers: heart rate and blood pressure, titration tolerance, and no abrupt discontinuation.
  • Mineralocorticoid-receptor antagonists (spironolactone, eplerenone): potassium and renal function.
  • SGLT2 inhibitors: volume status, genitourinary infection, and awareness of euglycemic diabetic ketoacidosis.
