5. Skilled interventions and teaching

Skilled interventions

  • Administer and monitor IV fluids per order with site and response assessment; track intake/output and weight against the parameters.
  • Provide skilled recognition of dehydration or over-hydration with parameter-based escalation and fall-safety measures.
  • Address contributing factors (fever, diarrhea, reduced intake) and coordinate electrolyte/renal labs as ordered.

Teaching / training (to mastery)

  • Teach the resident/representative the hydration goal, the signs of dehydration, and any fluid restriction (for HF/renal), with teach-back.
  • Train CNAs on accurate intake/output, offering fluids per the plan, and reporting reduced intake or dehydration/overload signs, with documented understanding.
