6. Red flags — escalate / transfer

*🚩 Red flags*
*Severe dehydration / hypovolemia* *— hypotension, marked tachycardia, oliguria/anuria, or acute confusion — escalate/transfer; sepsis signs → transfer.* *Acute kidney injury signs* *— a sharp drop in urine output with a rising creatinine — escalate (route to Genitourinary/Renal).* *Fluid overload / pulmonary edema* *from over-hydration — severe dyspnea, hypoxia, crackles — hold fluids per order and escalate/transfer (route to CHF overlay).* *Severe electrolyte disturbance* *— symptomatic hyper/hyponatremia or hyper/hypokalemia (confusion, weakness, arrhythmia, seizure) — escalate/transfer.*

Acute kidney injury signs — a sharp drop in urine output with a rising creatinine — escalate (route to Genitourinary/Renal).

Fluid overload / pulmonary edema from over-hydration — severe dyspnea, hypoxia, crackles — hold fluids per order and escalate/transfer (route to CHF overlay).

Severe electrolyte disturbance — symptomatic hyper/hyponatremia or hyper/hypokalemia (confusion, weakness, arrhythmia, seizure) — escalate/transfer.
