6. Red flags — escalate / transfer

🚩 Red flags

Surgical-site or deep infection / sepsis — spreading erythema, purulence, fever, or a wound that opens with systemic signs — escalate; sepsis → transfer (route to the Sepsis overlay).

DVT / PE — unilateral calf swelling/pain, or sudden dyspnea/pleuritic pain/hypoxia — escalate/transfer (time-critical).

Post-operative hemorrhage — excessive bleeding, a rapidly falling hemoglobin, or hemodynamic change — escalate/transfer.

Wound dehiscence/evisceration or ileus/obstruction — a wound that opens (cover with saline gauze) or absent bowel sounds with distension/vomiting — escalate/transfer.
