Vital signs are the backbone of skilled observation, but a set of numbers is not by itself skilled — what makes vital-sign monitoring a skilled service is the licensed judgment applied to it: trending against the ordered parameters, interpreting an abnormal value in the resident’s context, and recognizing the early trend that signals deterioration. This module supplies the cross-cutting technique so that every clinical reference and overlay can point to defensible vital-sign documentation.
The purpose is to prove, on the days parameter-based vital-sign surveillance is required, that the monitoring reflected skilled observation and assessment — not a routine task — under Pub. 100-02 Chapter 8. Vital-sign technique is the same as in home care; the coverage spine is re-pointed, and the parameters and escalation are made explicit.
