7. Skilled-need justification (SNF)

Parameter-based vital-sign surveillance is the mechanism of skilled observation; interpreting the values against parameters and recognizing an early deteriorating trend requires nursing judgment and is needed daily when a condition is unstable.

Correlating vital-sign trends with the plan and adjusting monitoring and escalation are skilled activities.
