5. Skilled interventions and teaching

Skilled interventions

  • Obtain vital signs on the ordered frequency with correct technique and document them against the parameters.
  • Trend the values and interpret the pattern in the resident’s context; act on an abnormal value or an early deteriorating trend.
  • Escalate per the notify parameters with a closed loop and adjust monitoring frequency as indicated.

Teaching / training (to mastery)

  • Teach the resident/representative any self-monitoring (e.g., home BP or glucose where relevant) and the values that prompt a call, with teach-back.
  • Train CNAs on accurate vital-sign technique and the immediate reporting of values outside set parameters, with documented understanding.
