MDS & care-plan linkage Oxygen, suctioning, tracheostomy care, ventilator, and isolation are coded in Section O0110; the record must document the setting, days, and procedure so the MDS is supported (F641). Each note ties to the respiratory care-plan problem/goal (baseline F655 → comprehensive F656 → revision F657); a respiratory decline may trigger a Significant Change assessment (F637).
