1. Purpose and clinical relevance

A change in a resident’s condition is a critical safety moment, and the failure to recognize, communicate, or act on it is a common source of harm and citations. The facility must immediately consult the physician and notify the resident’s representative when there is a significant change, a need to alter treatment, or a clinical complication (F580).

The documentation must show structured recognition (a baseline-to-current comparison), the SBAR to the provider, the physician and representative notifications with times, the interventions and monitoring, any transfer documentation, and whether the change triggers a Significant Change assessment.
