MDS & care-plan linkage A significant, non-self-limiting change in two or more areas triggers a Significant Change assessment within 14 days (F637); a transfer/return updates Section A entry/discharge items; the record must support the trigger and dates (F641). Each note ties to the relevant care-plan problem/goal, and the plan is revised after the change (F657).
