Recognize and quantify the change (baseline-to-current) and initiate the appropriate assessment and interventions.
Communicate via SBAR to the provider and notify the resident representative (F580); document times, content, orders, and follow-through.
Manage a transfer with a complete packet when needed, and evaluate for a Significant Change assessment; revise the care plan.
Teaching / training (to mastery)
Inform and involve the resident/representative in the change and the plan, and confirm understanding (teach-back), including advance-directive/code-status confirmation.
Train CNAs to recognize and promptly report early changes (intake, behavior, vitals, function), with documented understanding.
