Skilled interventions

  • Monitor glucose per order, administer insulin with sliding-scale/titration judgment, and document the response.
  • Recognize and treat hypo/hyperglycemia per protocol with a documented recheck; coordinate labs (ketones, electrolytes) as ordered.
  • Coordinate the insulin plan with intake and activity; manage thyroid/electrolyte monitoring where applicable.

Teaching / training (to mastery)

  • Teach the resident/representative glucometer use, insulin technique, the hypoglycemia rule (e.g., 15-15), sick-day guidance, and warning signs, with return-demonstration/teach-back.
  • Train CNAs on hypoglycemia recognition, timely meal delivery relative to insulin, and prompt reporting, with documented understanding.
