Daily-note bank — routine skilled days

Scenario

Model daily note (edit to the shift)

Post-urosepsis recovery

Day 4 on IV antibiotics: afebrile, HR/BP normalized vs the septic peak, mentation returning toward baseline, intake improving, IV antibiotic per order via a patent line, the treated UTI resolving, no new systemic signs. Skilled recovery management and recurrence surveillance required; supports Section I/O/N. (Continence reference.)

High-risk sepsis surveillance

At-risk resident (recent infection/indwelling device) monitored with parameter-based vitals and a mentation check, afebrile with no early sepsis signs, device/source assessed. Skilled early-sepsis surveillance required (time-critical); supports Section I. (General/Constitutional.)

Source-controlled sepsis, improving

Sepsis with the source controlled (e.g., catheter changed/removed, wound drained) and the systemic picture improving on IV antibiotics, vitals trending to normal. Skilled management and surveillance required; supports Section I/O.
