MDS & care-plan linkage Sepsis and the source (Section I), IV medication/isolation (Section O0110), and antibiotics (Section N) are coded from the record; the note must support each (F641). Each note ties to the sepsis/infection care-plan problem/goal (baseline F655 → comprehensive F656 → revision F657); sepsis or a septic deterioration triggers a Significant Change assessment (F637).
