Evolving sepsis — fever/hypothermia with tachycardia, tachypnea, or new confusion and a source — initiate the sepsis pathway, cultures, fluids, and empiric antibiotics without delay.

Source-specific emergency — an obstructed infected urinary system, necrotizing wound infection, or line sepsis — urgent source control and escalation.

Recurrence in recovery — a return of fever, tachycardia, or confusion during recovery — re-escalate immediately.
