Mucositis management, chemotherapy-related grade 2: the oral mucosa with patchy erythema and a few small ulcers, pain controlled to allow intake with the ordered oral care regimen (a bland rinse and topical analgesic) and a modified diet, no signs of oral infection, and intake maintained at an acceptable level. Licensed assessment of the mucositis grade and its intake/infection impact and delivery of the specialized oral care required; a daily skilled need. Supports Section L/K and the care-plan goal. Skilled oral care for the dependent/tube-fed resident: thorough oral care delivered per the aspiration-pneumonia-prevention protocol, the mucosa moist and intact, no thrush or lesions, and the mouth clear of secretions/debris. Skilled oral care and assessment required as a recognized measure to reduce aspiration pneumonia in the dependent resident; supports Section L and the goal.
