The skilled work spans the whole deficit set: rehabilitation-nursing carryover of the therapy plan for motor, sensory, swallowing, communication, cognitive, and visual deficits; the management of secondary prevention (blood pressure, antithrombotic therapy — antiplatelet or anticoagulation for a cardioembolic stroke, a statin, and glucose control); and surveillance for the stroke complications that drive readmission (aspiration, DVT, post-stroke depression, and a recurrent stroke).
This overlay proves, for each billed day, that skilled rehabilitation nursing, complication observation, management, or secondary-prevention teaching was reasonable, necessary, needed daily, and available only in a SNF under Pub. 100-02 Chapter 8.
